This form of lymphoma can present in a variety of clinical forms and may involve various organs, including the skin, lymph nodes, and viscera. PTCL often presents with an aggressive course and has a poor survival rate compared to other types of lymphoma, such as B-cell lymphomas. Key features of the disease are the variability of the clinical presentation, the presence of different subtypes, and the difficulty in diagnosis and treatment.

<div id="ez-toc-container" of the disease and interesting historical facts

The history of peripheral T-cell lymphoma began in the mid-20th century, when researchers began to differentiate lymphoma subtypes and classify them based on their cellular composition. In 1975, the first classification was proposed, which included separating T-cell lymphomas from B-cell lymphomas. An important step in the study of PTCL was the use of immunohistochemistry to determine the cell type, which significantly improved diagnostics. During this time, various subtypes were described, such as angiosarcoma, Sezary lymphoma, and others, which also contributed to a better understanding of these diseases. As molecular genetics developed, various mutations and abnormalities were identified, confirming the complexity and diversity of PTCL. Each discovery in this area helped to form a deeper understanding of the pathogenesis of the disease and became the basis for the development of targeted therapies.

Epidemiology

According to statistics, PTCL accounts for approximately 10-15% of all non-Hodgkin lymphoma cases. The incidence rate varies by geographic region and gender. The average age at diagnosis is 55-60 years, but PTCL can also occur in younger patients. Studies report that the incidence is higher in men than in women, with a ratio of 2:1. Given that lymphomas often occur in the setting of immunodeficiency, some studies show an increased incidence among patients with HIV/AIDS and other conditions that weaken the immune system. There has been an increase in the incidence of PTCL in recent decades, likely due to improved diagnosis and increased awareness.

Genetic predisposition to this disease

Research shows that certain genetic mutations and chromosomal abnormalities play a key role in the pathogenesis of peripheral T-cell lymphoma. Among the most frequently observed changes are mutations in genes associated with cell cycle regulation, as well as T-cell proliferation and differentiation. Among them are:

Mutations in the TP53 gene, which is responsible for cell cycle control

Chromosomal deletion 5q

Dysregulation of BCL-2 and BCL-6 genes

Abnormalities in chromosome 8q24 associated with MYC

Genetic changes associated with TCR (T-cell receptor)

Understanding genetic changes is important for developing targeted therapy and predicting the course of the disease. Thus, genetic predisposition plays a significant role in the occurrence and progression of PTCL.

Risk factors for the development of this disease

Risk factors for developing peripheral T-cell lymphoma are varied and may include physical and chemical exposures as well as certain medical conditions.
