This condition can manifest itself in tolerable or acute pain, a burning sensation, tingling or numbness in the perineum, as well as dysfunction of the urinary and reproductive systems. These symptoms can significantly reduce the quality of life of patients, leading to social isolation and psychoemotional disorders. Pudendal neuralgia is often associated with a number of different factors, such as injuries, pelvic surgeries and obstructive processes, which necessitates a comprehensive approach to the diagnosis and treatment of this disease.

<div of the disease and interesting historical facts

The history of pudendal neuralgia goes back to ancient times, but its clear description became possible only in the 20th century due to the development of anatomy and neurophysiology. The first mention of pain in the perineal region, which may indicate a problem with the pudendal nerve, is recorded in the works of ancient doctors such as Hippocrates. Later, in the early 19th century, the Scottish anatomist John Byron O'Shea spoke about the importance of studying the nerve as a key element in the diagnosis of perineal pain. In the 20th century, it became possible to use neuroimaging to study the state of the pudendal nerve, which opens up new horizons in understanding such a complex condition as neuralgia.

Epidemiology

Epidemiological studies of pudendal neuralgia show that this disease is relatively rare, but the exact prevalence remains unclear. According to various sources, neuralgia may be observed in approximately 1-2% of the general population, but such figures may vary significantly depending on the region and diagnostic methods. It is assumed that the disease is more common in women, especially during pregnancy and postpartum recovery. About 30% cases are associated with postoperative pain, which emphasizes the importance of prevention in surgical practice. These data indicate the need for more extensive research to identify factors contributing to the occurrence of this disease, as well as to improve diagnosis and treatment.

Genetic predisposition to this disease

Although the specific genetic causes of pudendal neuralgia are not yet fully understood, it is believed that susceptibility may be linked to several genes involved in nerve tissue repair and injury response. Some studies have suggested a possible link between neuralgia and mutations in genes involved in synaptic transmission of nerve impulses and myelination of nerve fibers. However, the exact mechanisms and types of mutations require further study, paving the way for future genetic research and the development of individualized treatment approaches.

Risk factors for the development of this disease

There are many factors that can contribute to the development of pudendal neuralgia, and they can be classified into physical, chemical, and associated diseases, including:

Pelvic injuries (eg, fractures or head injuries)

Surgical interventions in the pelvic area (eg, pelvic surgeries)

Diseases that lead to irritation of the nerves (eg, diabetes mellitus, viral neuritis)

Chemical exposures (e.g. alcohol or heavy metal intoxication)

Age-related changes associated with degenerative processes in the nervous system

Awareness of these risk factors may facilitate early diagnosis and prevention of pudendal neuralgia.

Diagnosis of this disease

Diagnosis of pudendal neuralgia is a multifaceted process that includes both clinical and instrumental methods of examination.
