This type of cancer is usually classified by stages, where the tumor invades the muscle layer, which significantly worsens the prognosis and requires a more intensive treatment approach. From the time of diagnosis of MIBC, the involvement of adjacent tissues and the presence of metastases to lymph nodes and other organs often become a subject of discussion during the development of an effective treatment strategy. Muscle-invasive bladder cancer is associated with multiple factors, including smoking, chemical exposure and chemotherapeutic agents. To date, despite ongoing efforts to understand the mechanisms of the disease, the survival rate of patients with MIBC remains quite low, which emphasizes the importance of early diagnosis and individualized treatment.

<div of the disease and interesting historical facts

The history of bladder cancer research dates back to ancient times. As early as 2000 BC, cases of bladder diseases were documented in Egypt. However, more detailed research began during the Renaissance, when scientists began to pay attention to anatomical and pathological changes. In the 19th century, a connection was identified between professions related to dye production and the development of bladder cancer. These scientific discoveries became the basis for further research into the etiology of the disease and its development.

Epidemiology

Overall, muscle-invasive bladder cancer occurs at a rate of 3-5 cases per 100,000 people per year. According to the latest data, approximately 200,000 new cases are diagnosed worldwide each year, with the disease being diagnosed more often in men than in women (ratio 3:1). The age groups most susceptible to the disease are people over 65 years of age. Research data show that the lifetime probability of developing MIBC in men is about 1 in 26, while in women this figure is lower - 1 in 88. Notably, in some regions, such as North America and Western Europe, the incidence is higher, which is associated with the prevalence of risk factors.

Genetic predisposition to this disease

Research shows that certain genetic mutations play an important role in the pathogenesis of muscle-invasive bladder cancer. The key genes involved are TP53, PTEN, FGFR3, and RB1. Mutations in the TP53 and RB1 genes are the most common and are associated with an aggressive course of the disease. In addition, in recent years, polymorphisms associated with the metabolism of chemicals and smoking have been identified that can increase the likelihood of developing MIBC. According to a study published in the journal Cancer Research, patients with a mutation in the FGFR3 gene are less likely to have disease progression.

Risk factors for the development of this disease

Risk factors that contribute to the development of muscle-invasive bladder cancer can be divided into several groups:

Physical factors: high levels of exposure to ultraviolet radiation, bladder trauma.

Chemical factors: contact with aromatic amines used in the production of dyes, rubber, as well as exposure to cadmium and arsenic.

Medical factors: previous chemotherapy with cyclophosphamide or pelvic radiation.

Lifestyle: Smoking is one of the main factors that increases the risk of developing the disease, increased alcohol consumption may also have an impact.

Chronic diseases: certain infectious pathologies, such as chronic cystitis, may predispose to the development of a tumor.

Diagnosis of this disease

The main symptoms of muscle-invasive bladder cancer include:

Hematuria (blood in the urine), which may be visible or microscopic.

Frequent urination accompanied by discomfort.

Pain in the lumbar and pelvic region.

Fatigue and weight loss in later stages of the disease.

Basic laboratory tests for diagnosis include urine analysis for abnormal cells, biopsy, and cytology.
