This condition most often occurs in the presence of a gastrinoma, a tumor, usually malignant, that develops in the pancreas or duodenum. Symptoms of the syndrome include gastritis, peptic ulcer disease, diarrhea, and other disorders associated with high acidity. of the disease and interesting historical facts

Kuster syndrome was described in detail in 1955 by American endocrinologist Richard Kuster. The condition was named after him after he identified a link between gastrin and the development of gastroduodenal ulcers. At the beginning of the study, the role of gastrin in the pathogenesis of peptic ulcer disease remained unclear, although there was already evidence of a link between increased acid secretion and gastrointestinal diseases. Over the next few decades, as technology advanced, the amount of data on the pathophysiology and genetic factors that play a role in the development of this syndrome rapidly increased.

Epidemiology

Kuster syndrome remains a rare disorder. Estimates of its incidence vary, but overall, available data suggest that gastronomia occurs in 1-3 people per million population each year. There is increasing interest in studying the syndrome, particularly as the incidence of gastroduodenal disorders in general increases. Most cases are diagnosed in adults over 30 years of age, and men are affected 2-3 times more often than women.

Genetic predisposition to this disease

It has been established that in some cases, Kuster syndrome may be associated with genetic mutations, particularly in patients with multiple endocrine neoplasia type 1 (MEN1), which is associated with certain mutations in the MEN1 gene. Mutations in other genes, such as the TSC2 gene, may also be involved. However, genetic predisposition is not the only factor, and further research continues to reveal complex interactions between hereditary and exogenous factors.

Risk factors for the development of this disease

Among the main risk factors contributing to the development of Kuster syndrome are:

The presence of gastrinoma, tumors of the pancreas or duodenum;

Family history of multiple endocrine neoplasia;

Pathologies such as chronic gastritis or peptic ulcer;

Chemical factors, including exposure to carcinogens;

Some environmental factors such as diet, stress levels and smoking.

Diagnosis of this disease

Various methods are used to diagnose Kuster syndrome:

The main symptoms include: abdominal pain, heartburn, diarrhea, weight loss and anemia;

Laboratory tests: blood test for gastrin levels, which will be significantly elevated in these pathologies;

Radiological examinations: ultrasound, CT or MRI to detect tumors;

Other types of diagnostics: endoscopic examination to assess the condition of the stomach and duodenum;

Differential diagnosis includes exclusion of other causes of hyperacidity, such as Zollinger-Ellison syndrome and chronic gastritis.

Treatment

Treatment of Kuster syndrome should be comprehensive:

General treatment may include gastroprotectors and antisecretory drugs;

Pharmacological treatment - use of proton pump inhibitors to reduce acid levels;

Surgery may be necessary to remove a gastrinoma if a resectable tumor is present;

Other treatments include chemotherapy and radiation therapy for metastases.

List of medications used to treat this disease

Among the medications recommended for the treatment of Kuster syndrome are:

Omeprazole;

Lansoprazole;

Pantoprazole;

Rabeprazole;

Dextromethorphan (for diarrhea).

Disease monitoring

Monitoring for Kuster syndrome requires regular screening:

Control steps include periodic measurement of gastrin levels;

The prognosis with adequate therapy can be favorable, however, in advanced cases, complications may develop;

Complications may include gastric perforation, severe anemia, and metastatic disease.

Age-related features of the disease

Kuster syndrome can manifest itself differently in different age groups.
